If you are a paediatrician or a paediatric subspecialist in active practice working in Canada, and you do not already receive the Canadian Paediatric Surveillance Program (CPSP) monthly case report forms, please join us by signing up below for monthly online reporting.

The reporting process is quick and simple:

• A hyperlink to a secured site is sent to you each month.
• No log-in or password is required.
• You can report from anywhere you have Internet access.
• You will have instant access to case definitions and full study protocols.

We take privacy seriously. Data is fully encrypted, and transmitted and hosted securely in Canada.

Because the CPSP conducts surveillance on rare paediatric diseases and conditions, the vast majority of participants report having seen no cases each month. If you do report a case, a follow-up clinical questionnaire will be sent to you for completion.
